Juan Maldonado-Reyes, 36, White Salmon, was transported by ambulance to Skyline Hospital in White Salmon following a one-vehicle accident.
The accident occurred on Nov. 3, 2004, at 8 a.m. on State Route 141 at milepost 17.95.
According to the report, Maldonado-Reyes was driving his 1999 Nissan Pathfinder northbound on SR 141 on black ice. The vehicle left the roadway to the right, hitting a rock embankment. The vehicle then rotated clockwise, re-entering the highway and coming to rest blocking the southbound lane.
Maldonado-Reyes was wearing a seatbelt at the time of the accident. He was cited for no insurance.
A 55-year-old Washougal man was taken by Life Flight to Emanuel Hospital in Portland, following a one-vehicle accident on SR 14 at milepost 46.65.
According to the report, the accident occurred at 4:23 p.m. on Nov. 6. The report stated Donald M. Wilsey was driving his 1999 Ford pickup westbound on SR 14. The vehicle crossed the westbound fog line. The driver overcorrected, causing the vehicle to fish tail and cross the centerline. The vehicle came back across the westbound lane, striking the jersey barrier and rolling onto the driver's side. The vehicle came to a sliding stop in the westbound lane.
The roadway was blocked for one hour.
The driver was wearing a seatbelt at the time of the accident.
