A Toledo man was taken by Life Flight following a two-vehicle accident in Goldendale. The accident occurred on Sept. 25, 2008, at 9:55 a.m. at the intersection of E. Simcoe Drive and S. Columbus Ave.

According to the report, Peter E. Dennis, 41, White Salmon, was driving his 1977 Peterbilt semi northbound on S. Columbus approaching the intersection with Simcoe Dr. A 1986 Pontiac driven by Donald C. Smith, 67, Toledo, was southbound on S. Columbus, also approaching Simcoe Dr. intersection. Smith attempted a left turn into the front of Dennis's vehicle.

Smith was taken by Life Flight to Emanuel Hospital in Portland with head and cervical injuries.

Dennis was not injured.

Smith was charged with failure to yield right of way.

A California man was killed in a one-vehicle accident on State Route 14 at milepost 93.7, 12 miles southwest of Goldendale. The accident occurred on Sept. 23, 2008, at 4:28 p.m.

According to the report, Singh Harjinder, 52, Elk Grove, Calif., was driving his 2001 Freightliner semi westbound on SR 14, through the Wishram Curves. The driver entered the westbound ditch, corrected to the left. The vehicle crossed the eastbound lane, broke through the eastbound guardrail and rolled approximately 200 feet down an embankment.

Harjinder was pronounced dead at the scene.

Cause of the accident is still under investigation.

Two people were injured in a three-car accident on SR 14 at milepost 36 in North Bonneville. The accident occurred on Sept. 29, 2008, at 1:40 p.m.

According to the report, a 1995 Volvo driven by Jason R. Randles, 31, Washougal, was westbound on SR 14 following a 1995 Mitsubishi Montero driven by Michael L. Willman, 28, Vancouver. The two vehicles stopped in the roadway for a flagger in a signed construction zone. A 2003 Dodge Caravan driven by Shilrye M. Morford, 62,, Vancouver, was westbound on SR 14. Morford's vehicle struck the rear end of the Mitsubishi which was pushed into the Volvo.

Morford suffered possible neck and back injury was transported by aid car to Southwest Washington Medical Center in Vancouver. Randles had forehead contusion, hip abrasion and possible neck injury. He was also transported by aid car to Southwest Washington Medical Center. willman was not injured.

Randles was cited for DUI.
